W-2 Housing Allowance

We made a serious error on our minister's W-2 for 2012 and are trying to correct it by putting the right numbers in the right boxes.

His housing allowance according to the adopted resolution was $30,000. However, due to the recession we were only able to pay him a total of $23,000 and his housing expenses were only $18,000. This leaves a taxable income of $5000.

Which of these numbers do we put in which boxes?
