First Lady was reimbursed for healthcare premium in 2012. The total is over $3900. She is not an employee of the church so I can not W-2 her. Do I 1099-misc?

If we pay the pastor's wifes' health insurance directly to the insurance company do any end of the year forms need to be filled out for her to file? I have bee putting this under "pastorial support" but am being asked to change the category to health insurance - pastor's wife. She is not an employee of the church.
