I read the post about the LETTER that the pastor is given to confirm his housing allowance. But the post said that this letter is NOT sent in with the tax return. What form must be sent with the pastor's income tax return when noting the housing allowance?

It rest with the minister to be fair about his/her housing allowance since nothing is sent to the IRS that confirms it to them. As well, it is optional that the church office could put the housing allowance amount on the pastor's W-2 form in Box 14.
