We had a missionary returning home to leave the mission field. We had an old house that had not been lived in for 10 years and allowed him to live there for 3 months and did not charge him. Do we have to issue him a a 1099 form?

This sounds like a nonmonetary gift to me, and would not necessitate a 1099 form. You might have him/her to call the 1-800-1040 tax information number and ask about any questions he/she may have about income tax preparation.
