Our pastor is the only pastor and is probably considered an employee, although we haven't made an official decision on it during council or anything. However, he has opted out of SS tax. Should we only fill out the wages/housing allowance portion on his W-2?

You would fill it out just as you would for a minister that has not opted out of Social Security.
