If a minister chooses to Opt Out of Social Security with IRS using Form 4361, does this mean he will not receive SS at retirement age?

What if the pastor has paid social security taxes from other jobs

Does he lose those benefits if form 4361 is approved?

That's right. He/she will plan their own retirement--not many vocations are given that option.
