Musical Instruments donated to our church

We have in the past had members donate stuff to the church, but when they leave, they claim it back. Is there a form that we could get them to complete when donations of this kind are made so that when they leave, the assets stay with the church? Can anyone maybe send me a draft of such a form?

Donations or Loans???

Seems to me that the easiest thing to do would be to make up a simple form that would identify the donor, identify the specific instrument, and have a place to check off if the instrument is being donated or being loaned.

If the donor / loaner indicates that the instrument is being donated, then there should be a clear statement indicating that the donor will not be able to reclaim the instrument and that the church may either keep the instrument, sell it or do whatever the church chooses to do with it.

If the donor / loaner indicates that the instrument is being loaned to the church, then there should be a statement indicating that the instrument is to be returned to the loaner upon that individuals written request.

The main point is that whether it is being donated or being loaned should be determined at the outset so that there will be no confusion in the future. It will be very important to safeguard these documents in a file in the church office well into the future.
