How do I fill out the W-2 if the pastor has opted out of SS tax?

Our pastor is the only pastor and is probably considered an employee, although we haven't made an official decision on it during council or anything. However, he has opted out of SS tax. Should we only fill out the wages/housing allowance portion on his W-2?

W-2 for Minister Who Has Opted Out of Social Security

You would fill it out just as you would for a minister that has not opted out of Social Security.
See this page for an example of a W-2 for a minister.

On the minister's personal return is where an approved IRS Form 4361 would be recorded.
