Freebies for Your Church or Nonprofit

You will find a lot of these downloads scattered among my site. For your convenience, I have put all of them on one page and will also be putting more free stuff up from time to time so check back often.

Contributions:

This is a really detailed example of an annual contribution statement. Yours doesn't have to be this detailed. It only has to include three important details. See what your church receipt must contain here. However, this will give you a general idea what one looks like:

Annual Contribution Statement

This is an example of a noncash contribution statement:

You will need Adobe Reader (the latest version is recommended) installed on your computer in order to save or open any of these pdf files.

You can get Adobe Reader free here (a new window will open so you can download it without leaving this page).

If you want to open the file in your browser window, just click on the link . However, if you want to download the file to view later, then right-click on the link and choose "Save Target As" or "Save File As". Then select where you want to save the file on your hard drive.

If you have any trouble modifying the form, you can convert a pdf file to a Word file or convert a Word file to a pdf file to easily modify on this cool site.

Payroll Forms:

If you have employees, you need to have a completed W-4 form for each employee on file.

IRS Form W-4

For every non employee that you paid more than $600 in a year, you should have a completed W-9 on file.

IRS Form W-9

You can NOT download a usable 1099-MISC to send to those non employees, but here is a sample of this form and the number to call to get your 1099s and 1098s.

Example of a 1099-MISC

The same thing goes for the W-2s you will need for your employees.

Example of a W-2

For the fun part of sending those employee's payroll taxes in, here is an Employer's Quarterly Report:

IRS Form 941

Miscellaneous:

For your Pastor or staff mileage reimbursment:

For a internal audit...a checklist with a places to check off and initial each entry:
